Paha EBOLA virus tullut pahemmaksi (1.8.2014)

Ebolassa on käsittääkseni monosyyttien aktivoima  kudoksen proteolyyttisen kaskadin  progredioituva aktivoituminen, mutta siihen ei ole  vastaavaa progredioivaa lääkitystä olemassa.  monosyyttien hiljentäjää,  TIMP - kaltaista MMP( et al)  proteolyyttisen kaskadin  jarruttajaa ja ECM stabiloijaa.   Ehkä suuri määrä kortisonia auttaa stabiloimaan kalvoja, sellainen septisen shokin annos. Lisäksi kehon jäähdytys  voi auttaa  hidastamaan proteolyysiä.  Hibernaatiossa ( histamiinin vapautumisen estoa) aivorasvojen puolella  on neurolepteistä apua.
On mahdollisuuksia rokotekehittelyyn kertoo seuraava tieto.

Hepatiittiviruksen HBc nukleokapsidiproteiini ja varhainen e antigeeni, HBeAg

Hepatitis B Core and e Proteins
The core ORF encodes two sequence related yet functionally distinct proteins: the hepatitis B core protein and the hepatitis B e protein.

The hepatitis B core protein (HBc) is the major component of the nucleocapsid shell packaging the HBV genome. This 185 amino acid protein is expressed in the cytoplasm of infected cells. Sequence analysis indicates that HBc is predominantly composed of hydrophilic and charged amino acids. This protein remains non-glycosylated and is not modified through the addition of lipids.
The region of HBc associated with nucleocapsid assembly lies within the first 149 amino acids. (1) More recently, the crystal structure of the nucleocapsid has been resolved, giving more insight into the structure of the HBc protein. (2) HBc is largely a helical protein with five alpha-helices. This is unusual in that most nucleocapsid proteins which form icosahedral shells typically fold into barrel structures such as those seen in the HIV-1 capsid. (3), (4) A long alpha-helical hairpin dominates the folding pattern of HBc, consisting of helices 2 and 3. A hydrophobic core makes up the base of the protein.
HBc dimers, required for the formation of the nucleocapsid, associate via their alpha-helical hairpins which form a four-helix bundle. (5) A disulfide bridge on the dimer interface between the Cys-61 residues forms to stabilize the association. (6), (7) However, this residue is not critical as its deletion does not impair dimer formation. (8), (9), (10) Dimer associations to produce the nucleocapsid shell appear to involve residues Tyr-132, Arg-127, Pro-129 and Ile-139. (11)
The C-terminal end of the protein contains four arginine clusters which appear to be involved in nucleotide packaging. (12), (13) HBc can also be phosphorylated (14), (15) on serine residues at positions 157, 164 and 172. Phosphorylation at series 164 and 172 impairs pgRNA packaging. (16), (17)
HBc is able to form particles with outward pointing spikes even if a foreign peptide sequence is spliced into the HBc coding region.
As such, HBc can be used for displaying foreign protein domains on the surface of HBV nucleocapsids. (18)

Unlike the core protein, the hepatitis B e antigen has a different fate. The e antigen is so named due to its "early" appearance during an acute HBV infection. HBe is expressed by the translation of sequences upstream of the HBc ORF known as the pre-C sequence. The pre-C sequence encodes a hydrophobic transmembrane domain, resulting in the translation/translocation of HBe into the lumen of the ER. (19) This fundamental change in the location of protein expression alters the antigenicity of HBe such that it does not share antigenic homology with HBc, despite having nearly identical amino acid sequences.
In the ER, 19 of the 29 residues of the pre-C region are removed by a signal peptidase.
The remaining pre-C residues prevent HBe from forming into core particles. (20) Another region in the arginine rich domain is cleaved from HBe in the Golgi complex. (21)
The purpose of producing this secretory form of HBc is not well understood.

One theory is that high levels of HBe may somehow suppress the immune system from eliminating HBV-producing cells. High levels of HBe are often found in the serum of highly viraemic HBV carriers. It has also been found that woodchucks do not undergo persistent WHV infection if a HBe-minus variant is used for infection. (22)l Host. J. Virol.; 66: 5682-5684.

HBV, uusi väitöskirja

Maailmassa potee 240  miljoonaa henkilöä HBV infektiota, joten se on merkittävä tauti.
Simon Larson on tehnyt väitöskirjansa aiheesta Hepatiitti B viruksen replikaatio ja integraatio. Väitöstilaisuus on ensi kuun alussa.
Väitöskirjaa voi edeltä  tutkia osoitteesta ISBN 978-91-628-8986-9

Hepatitis B virus replication and integration

Suomennosta abstraktista:
Alkuvaiheessa potilailla on  korkeat pitoisuudet hepatiitti B-viruksen DNA:ta veressään ja he ilmentävät  e antigeenia. (HBeAg).
Chronic infection with hepatitis B virus (HBV) affects 240 million people worldwide and may cause liver disease including hepatocellular carcinoma (HCC). Initially patients have high levels of HBV DNA in their blood, no liver disease and express the e antigen (HBeAg).
At some point an immune response is mounted and the viral load decreases with several log10 copies/ml, they lose HBeAg and severe liver damage may follow if the virus is not cleared efficiently
  • Samaan aikaan HBsAg-pitoisuudet, kiertävän pinta-antigeenin pitoisuudet, pysyvät korkeana. Tämä pinta-antigeeni ei ole kiinnittyneenä viruksiin.  Maksassa saattaa viruksen DNA integroitua maksan hepatosyyttisolujen genomiin. Sitä arvellaankin mekanismiksi, mikä mahdollisesti edistää syövän kehittymistä.
Meanwhile, the circulating levels of the surface antigen (HBsAg), not bound to viral particles, remain high. In the liver, the viral DNA might integrate in the genome of hepatocytes. This has been proposed as a mechanism potentially promoting cancer development
  • Tämän väitöskirjan tarkoituksena oli  tutkia sitä mekanismia, mistä johtuu  HBeAg:n kadotessa tapahtuva  suuri vähenemä HBV DNA määrässä,  kun  HbsAg-pitoisuudet pysyvät kuitenkin suhteellisen stabiileina , ja arvioida  seerumin  HBsAg pitoisuuden   määrittämisen käyttökelpoisuus maksavaurion merkitsijänä  sekä määrittää maksabiopsioista käsin  integroituneen HBV DNA:n  määrä.
The aims of this thesis were to investigate the mechanisms behind the great decline in HBV DNA at loss of HBeAg while the HBsAg levels remain relatively stable, to evaluate the utility of quantification of HBsAg in serum as a marker for liver damage, and to assess the extent of integrated HBV DNA in liver biopsies.
The main methods used in this thesis are various types of polymerase chain reaction (PCR). The material was blood samples and liver biopsies from chronic carriers of HBV. Viral load in the liver, both DNA and RNA, was quantified by real-time PCR and integrated HBV DNA sequences were identified using Alu-PCR.
  •  Tuloksista. Seerumin HBV DNA-pitoisuudet ja HBsAg -pitoisuudet korreloivat intrahepaattisten  (maksansisäisten) kovalentisti sulkeutuneitten DNA -rinkuloitten (ccc-DNA) määrään, jotka voivat toimia   templaattina uusille viruspartikkeleille ja antigeenille.
Serum levels of HBV DNA and HBsAg correlated with intrahepatic levels of covalently closed circular DNA (cccDNA), the template for new viral particles and antigens.
By comparing viral load between patients positive or negative for HBeAg it was found that the 3-5 log10 decline of HBV DNA at HBeAg seroconversion mainly is explained by decrease in cccDNA and reduced transcriptional efficiency of pregenomic RNA (pgRNA), the template for virus DNA.
However, retention of viral particles and decreased half-life of virions seem to have an additional impact.
  • Kun vertailtiin tuloksia seerumin  HBsAg -pitoisuuksista ja  maksabiopsiatutkimuksista  , tehtiin johtopäätös, että  raja-arvona alle 3 logaritmista  IU/ml yksikköä  HBsAg ja  alle 4 logaritmista  yksikköä viruskopioita /ml  voitaisiin tunnistaa 96%:n  spesifisyydellä   ne potilaat, joilla on  matala-asteinen maksavaurio
 By comparing results of serum levels of HBsAg and histological examination of liver biopsies it was concluded that a cut-off of <3 .0="" 96="" a="" and="" copies="" could="" damage="" hbsag="" identify="" iu="" liver="" log10="" low="" ml="" of="" patients="" span="" specificity="" with="">
  •   Integroituneita HBV DNA sekvenssejä voitiin tunnistaa  36 maksabiopsianäytteessä 48:sta näytteestä . Kaikenkaikkiaan analysoitiin 45 integroitunutta sekvenssiä 32 eri potilaalta.  HBV DNA-integroituminen maksasolugenomiin oli hyvin tavallinen tapahtuma B-hepatiittiviruksen  kroonisilla kantajilla
With Alu-PCR integrated sequences were detected in 36 of 48 liver biopsies examined. In total 45 integrated sequences were analysed from 32 different patients. Integration of HBV DNA was thus a very common event in the chronic HBV carriers.
  • Yhteenveto:  Tämä tutkimus  loi lisävaloa B-hepatiittiviruksen replikaation ja integraation käsittämiseen.
In summary, this study contributes to the understanding of the replication and integration of the hepatitis B virus

Det visar en studie som publicerats i Proceedings of the National Academy of Sciences (PNAS).
